I, hereby give permission for my child , to participate in the seasonal camp organized by Roots and Wings Camp. In consideration for my child being allowed to participate in the camp, I agree to the following terms and conditions:
1. Assumption of Risk: I understand that participating in camp activities involves inherent risks, including but not limited to, physical injury, illness, or property damage. I acknowledge that Roots and Wings Camp has taken reasonable measures to minimize such risks, but I understand that accidents or injuries may still occur.
2. Medical Authorization: In the event of an emergency, I authorize Roots and Wings Camp staff to seek and provide necessary medical treatment for my child. I understand that Roots and Wings Camp will make reasonable efforts to contact me or the emergency contact listed below prior to seeking medical treatment, but if they are unable to do so, I authorize them to act in the best interest of my child's health and safety.
3. Release of Liability: I hereby release, discharge, and hold harmless Roots and Wings Camp, its directors, officers, employees, volunteers, and agents from any and all claims, demands, causes of action, or liability arising out of or related to my child's participation in the camp, including but not limited to, personal injury, illness, or property damage.
4. Code of Conduct: I understand and agree that my child must adhere to the camp's rules and code of conduct. I acknowledge that Roots and Wings Camp] reserves the right to dismiss my child from the camp without refund if they engage in behavior that is deemed inappropriate or violates the camp's policies.
5. Parent/Guardian Responsibility: I understand that as the parent/guardian, I am responsible for providing accurate and up-to-date information regarding my child's medical history, allergies, medications, and emergency contact information.
6. Parents understand that a student’s attendance at summer camp is contingent upon the student meeting the school’s standards of acceptable behavior and citizenship. A student may be asked to leave our camp at the sole discretion of Roots and Wings Camps.
